Please submit all forms to your local association.
- 2025 CTCPA New Member Form for Unrated Members – New members must submit this form along with payment to their local association. Each individual is required to hold a valid membership with their local association.
Member Association Forms
Association Secretaries: Please submit these forms to the CTCPA Office.
- Sanctioned Show Application – 2025 – This form must be completed by each association and submitted at the beginning of the year.
- 2025 Affiliate Membership Summary – Association Secretaries must use this form when submitting affiliate membership forms and payments. Please include either the membership backup sheets for each individual member or the member information spreadsheet along with the summary.
- 2025 Participation and Levy Form – This form must be completed by each Show Operator or Association after every CTCPA-sanctioned show. It must be submitted along with participation fees within 21 days of the event. Failure to submit payment within this timeframe will result in the show not qualifying as a CTCPA-sanctioned event.
